Case Management Specialist

Supports the Case Management staff by providing clerical support for the department's Case Managers.

These tasks include but are not limited to: arranging post-discharge services, issuing letters, providing clinical updates to commercial payers, as well as communicating necessary discharge planning updates to the hospital care team staff.

Assures completeness of packet materials, communicates proposed discharge plan to family and extended care facility.

Incorporates age specific needs in discharge plan.

Prepares timely and smooth discharges.

Demonstrates a working understanding of federal and state regulations, departmental policies, particularly those related to discharge planning.

High School Diploma or equivalent Minimum of 1 year previous healthcare experience with knowledge of patient discharge planning procedures required.

M-F 9am to 5:30pm
