Under the supervision of the Chief Quality Officer (System), the Quality Coordinator plays a vital role in our healthcare delivery system. Primary areas of accountability may include:
- Creation and execution of hospital wide “Conversation Ready” Initiative
- Point person for any ACP/EOL needs on patients without Palliative Care on board
- Supportive role to all clinicians to ensure thorough ACP
- Supportive role to all clinicians to provide the highest level of care to patients nearing EOL.
- Ongoing training/involvement for designated “Conversation Champions”
- Creation of educational and interactive resources for patients and their families to assist with EOL decision making
- Identification of triggers to start palliative and hospice consultations earlier in the hospitalization
- Ongoing clinician palliative concepts education
- Conversation Ready educational classes
- Skills Day Conversation Ready education
- Development of Cerner documentation to capture ACP
- Development of note templates to capture ACP conversations ? Continued process improvement/growth of the initiative
- Data collection on impact of the initiative
- Assist with goals of GIP hospice initiative
- Nurse education
- Nurse GIP conversion protocol
- Hospice liaison
- Supportive role to bedside nurses to provide highest level of care to patients under to hospice care
