Job Summary:
- Under the direction of Case Management, serves as a coordinator of an interdisciplinary approach to guide patients through the continuum of care with special focus on discharge planning, psychosocial needs assessment, care/resource management, and provider/insurance issues.
- Collaborates with the physicians, patients, and their family members and others to facilitate the achievement of desired outcomes, address ongoing needs, maximize patient, family and provider satisfaction, optimize systems/resource utilization, facilitate timely discharge, assure appropriate length of stay, reduce unnecessary delays and other adverse variances to the delivery of services.
Minimal Acceptable Job Qualifications
Education:
- Master's degree in Social Work or a Human Services Field.
Experience:
- Minimum of one to two years of clinical social work experience in a hospital or health care setting which may have included the appropriate supervision or internship.
