Job Description

Job Summary

The LPN Graduate works under the supervision of a Registered Nurse (RN) or licensed healthcare provider to deliver patient care in accordance with established policies, procedures, and standards of care. While completing requirements for licensure as a Licensed Practical Nurse (LPN), the LPN Graduate gains hands-on experience and clinical skills in a supportive healthcare environment. This role focuses on basic nursing care, professional development, and collaboration with the healthcare team to ensure high-quality patient outcomes.

Essential Functions

  • Monitors and records patient vital signs, including blood pressure, pulse, temperature, and oxygen saturation, reporting any changes or concerns to the supervising RN or physician.
  • Assists in patient assessments, documenting observations in the electronic health record (EHR) and communicating findings to the clinical team.
  • Supports patient mobility by assisting with ambulation, range of motion exercises, and other physical activities according to care plans.
  • Administers medications and treatments under supervision, ensuring adherence to physician orders, protocols, and safety guidelines while maintaining accurate documentation.
  • Prepares patients for medical procedures, tests, or surgeries, providing emotional support and patient education as directed by the care team.
  • Collaborates with RNs, physicians, and interdisciplinary team members to deliver coordinated, comprehensive patient care.
  • Ensures a clean and safe environment by adhering to infection prevention and control standards, including proper handling and disposal of medical waste.
  • Educates patients and families on care plans, medication management, and health practices within the scope of practice and under supervision.
  • Participates in clinical education and training opportunities, developing skills and competencies required for LPN licensure.
  • Performs other duties as assigned.
  • Complies with all policies and standards.

Qualifications

  • Technical School with completion from a state approved, accredited Licensed Practical Nurse (LPN) program required and
  • Other NCLEX-PN exam eligibility or successful completion pending licensure required
  • 0-2 years of experience as a nursing assistant or other allied health role required

Knowledge, Skills and Abilities

  • Basic understanding of nursing principles and practices, including patient care techniques and safety protocols.
  • Strong communication and interpersonal skills to collaborate effectively with patients, families, and healthcare team members.
  • Attention to detail and ability to follow established procedures and clinical guidelines.
  • Proficiency in basic computer skills, including the use of electronic health records (EHR).
  • Willingness to learn and grow in a dynamic healthcare environment.

Licenses and Certifications

  • LPN - Licensed Practical Nurse - State Licensure preferred
  • BCLS - Basic Life Support issued by American Heart Association (AHA), American Red Cross (ARC), or American Safety and Health Institute (ASHI) required
